ABSTRACT

SUMMARY: Senile osteoporosis is mainly caused by reduced osteoblast differentiation and has become the leading cause of fractures in the elderly worldwide. Natural organics are emerging as a potential option for the prevention and treatment of osteoporosis. This study was designed to study the effect of resveratrol on osteogenic differentiation of bone marrow mesenchymal stem cells (BMSCs) in osteoporosis mice. A mouse model of osteoporosis was established by subcutaneous injection of dexamethasone and treated with resveratrol administered by gavage. In vivo and in vitro, we used western blot to detect protein expression, and evaluated osteogenic differentiation of BMSCs by detecting the expression of osteogenic differentiation related proteins, calcium deposition, ALP activity and osteocalcin content. Resveratrol treatment significantly increased the body weight of mice, the level of serum Ca2+, 25(OH)D and osteocalcin, ration of bone weight, bone volume/total volume, trabecular thickness, trabecular number, trabecular spacing and cortical thickness in osteoporosis mice. In BMSCs of osteoporosis mice, resveratrol treatment significantly increased the expression of Runx2, osterix (OSX) and osteocalcin (OCN) protein, the level of calcium deposition, ALP activity and osteocalcin content. In addition, resveratrol treatment also significantly increased the expression of SIRT1, p-PI3K / PI3K and p-AKT / AKT in BMSCs of osteoporosis mice. In vitro, resveratrol increased the expression of SIRT1, p-PI3K / PI3K and p-AKT / AKT, Runx2, OSX and OCN protein, the level of calcium deposition, ALP activity and osteocalcin content in BMSCs in a concentration-dependent manner, while SIRT1 knockdown significantly reversed the effect of resveratrol. Resveratrol can attenuate osteoporosis by promoting osteogenic differentiation of bone marrow mesenchymal stem cells, and the mechanism may be related to the regulation of SIRT1/PI3K/AKT pathway.


La osteoporosis senil es causada principalmente por una diferenciación reducida de osteoblastos y se ha convertido en la principal causa de fracturas en las personas mayores en todo el mundo. Los productos orgánicos naturales están surgiendo como una opción potencial para la prevención y el tratamiento de la osteoporosis. Este estudio fue diseñado para estudiar el efecto del resveratrol en la diferenciación osteogénica de las células madre mesenquimales de la médula ósea (BMSC) en ratones con osteoporosis. Se estableció un modelo de osteoporosis en ratones mediante inyección subcutánea de dexametasona y se trató con resveratrol administrado por sonda. In vivo e in vitro, utilizamos Western blot para detectar la expresión de proteínas y evaluamos la diferenciación osteogénica de BMSC detectando la expresión de proteínas relacionadas con la diferenciación osteogénica, la deposición de calcio, la actividad de ALP y el contenido de osteocalcina. El tratamiento con resveratrol aumentó significativamente el peso corporal de los ratones, el nivel sérico de Ca2+, 25(OH)D y osteocalcina, la proporción de peso óseo, el volumen óseo/ volumen total, el espesor trabecular, el número trabecular, el espaciado trabecular y el espesor cortical en ratones con osteoporosis. En BMSC de ratones con osteoporosis, el tratamiento con resveratrol aumentó significativamente la expresión de las proteínas Runx2, osterix (OSX) y osteocalcina (OCN), el nivel de deposición de calcio, la actividad de ALP y el contenido de osteocalcina. Además, el tratamiento con resveratrol también aumentó significativamente la expresión de SIRT1, p-PI3K/PI3K y p-AKT/AKT en BMSC de ratones con osteoporosis. In vitro, el resveratrol aumentó la expresión de las proteínas SIRT1, p-PI3K/PI3K y p- AKT/AKT, Runx2, OSX y OCN, el nivel de deposición de calcio, la actividad de ALP y el contenido de osteocalcina en BMSC de manera dependiente de la concentración, mientras que La caída de SIRT1 revirtió significativamente el efecto del resveratrol. El resveratrol puede atenuar la osteoporosis al promover la diferenciación osteogénica de las células madre mesenquimales de la médula ósea, y el mecanismo puede estar relacionado con la regulación de la vía SIRT1/PI3K/AKT.

ABSTRACT

Purpose: This study aimed to investigate the effect of virtual reality (VR) technology in children after surgery for concomitant strabismus. Methods: A total of 200 children with concomitant exotropia or concomitant esotropia were randomly divided into a training group and a control group according to the single even number random method (100 cases in each group). Patients in the training group received VR intervention training within 1 week after surgery. Patients in the control group did not receive any training. Results: Six months after the surgery, the orthophoria (the far or near strabismus degree was ?8?) rate was significantly higher in the training group than in the control group (P = 0.001), while the eye position regression rate (compared to the strabismus degree within 1 week after the surgery, the amount of regression >10?) was significantly lower in the training group than in the control group (P = 0.001). Six months after the surgery, the number of children with simultaneous vision and remote stereovision was significantly higher in the training group than in the control group (P = 0.017 and 0.002, respectively). The differences in the number of patients with peripheral stereopsis, macular stereopsis, and stereopsis in macular fovea centralis at 1, 3, and 6 months after the surgery between the training and the control groups were not statistically significant (P = 0.916, 0.274, and 0.302, respectively). Conclusion: The intervention of VR technology after strabismus correction effectively improved children’s visual function and maintained their eye position

ABSTRACT

OBJECTIVE To provide reference for the diagnosis and treatment of Stevens-Johnson syndrome caused by rebamipide, and to explore the predisposing factors of Stevens-Johnson syndrome. METHODS Clinical pharmacists analyzed the treatment process of a patient with gastrointestinal diseases and evaluated the correlation between the drug used and adverse reactions, in order to determine the suspected allergenic drug causing Stevens-Johnson syndrome. The predisposing factors of patients with Stevens-Johnson syndrome were explored. RESULTS & CONCLUSIONS The suspected allergenic drugs that caused the patient to develop Stevens-Johnson syndrome included Ilaprazole enteric-coated tablets, Rebamipide tablets and Kangfuxin liquid. In summary, the suspect drug was identified as Rebamipide tablets according to the causality evaluation method of the National Center for Adverse Drug Reaction Monitoring, Naranjo’s scoring method and the algorithm of drug causality for epidermal necrolysis scoring criteria. Hypoproteinemia, competitive binding of plasma proteins between drugs, advanced age, bacterial and viral infections were the predisposing factors of Stevens-Johnson syndrome. Therefore, before using rebamipide in clinical practice, it is necessary to inquire about the patient’s allergy history in detail. During the use process, it is necessary to strengthen the patient’s medication monitoring and be alert to the occurrence of serious adverse reactions. If any abnormalities are found, the medication should be stopped immediately and symptomatic treatment should be given as soon as possible to ensure the safety and effectiveness of the patient’s medication.

ABSTRACT

The theory of membrane anatomy has been widely used in the field of colorectal surgery. The key point to perform high quality total mesorectal excision (TME) and complete mesocolic excision (CME) is to identify the correct anatomical plane. Intraoperative identification of the various fasciae and fascial spaces is the key to accessing the correct surgical plane and surgical success. The landmark vessels refer to the small vessels that originate from the original peritoneum on the surface of the abdominal viscera during embryonic development and are produced by the fusion of the fascial space. From the point of view of embryonic development, the abdominopelvic fascial structure is a continuous unit, and the landmark vessels on its surface do not change morphologically with the fusion of fasciae and have a specific pattern. Drawing on previous literature and clinical surgical observations, we believe that tiny vessels could be used to identify various fused fasciae and anatomical planes. This is a specific example of membrane anatomical surgery.

ABSTRACT

Objective: To analyze the safety, effectiveness, economics, innovation, suitability and accessibility of tetrandrine in the treatment of pneumoconiosis, and provide evidence-based basis for health policy decision-making and clinical practice. Methods: In July 2022, the system searched PubMed, Embase, the Cochrane Library, CNKI, Wanfang, SinoMed databases (the retrieval time was from the establishment of the database to June 30, 2022), screened the documents that meet the standards, extracted and evaluated the data, and used the "HTA checklist" developed by the International Network of Agencies for Health Technology Assessment (INAHTA) to evaluate the HTA report. AMSTAR-2 Scale was used to evaluate the quality of systematic evaluation/Meta analysis. CHEERS Scale was used to evaluate the quality of pharmacoeconomics research. The included cohort study or case-control study was evaluated with the Newcastle-Ottawa Scale. The included randomized controlled trial (RCT) studies were evaluated using the Cochrane Risk Bias Assessment Tool (Cochrane RCT) quality evaluation criteria. Comprehensive comparison and analysis based on the characteristics of the data included in the study. Results: A total of 882 related literatures were detected from the initial screening. According to relevant standards, 8 RCT studies were finally selected for analysis. Statistical results showed that basic treatment with tetrandrine could better improve FEV(1) (MD=0.13, 95%CI: 0.06-0.20, P<0.001), FEV(1)/FVC (MD=4.48, 95%CI: 0.61-8.35, P=0.02) and clinical treatment efficiency. Tetrandrine had a low incidence of adverse reactions. The affordability coefficient of tetrandrine tablets was 0.295-0.492. Conclusion: Tetrandrine can improve the clinical symptoms and pulmonary ventilation function of pneumoconiosis patients, most of the adverse reactions are mild, and the clinical application is safe.

ABSTRACT

Coronavirus disease 2019 (COVID-19) has yet to be proven to alter male reproductive function, particularly in the majority of mild/asymptomatic patients. The purpose of this study was to explore whether mild/asymptomatic COVID-19 affects semen quality and sex-related hormone levels. To find suitable comparative studies, a systematic review and meta-analysis was done up to January 22, 2022, by using multiple databases (Web of Science, PubMed, and Embase). Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were used to identify and choose the studies. Meta-analysis was used to examine the semen parameters and sex-related hormones of mild/asymptomatic COVID-19 patients before and after infection. The effects of semen collection time, fever, and intensity of verification on semen following infection were also investigated. A total of 13 studies (n = 770) were included in the analysis, including three case-control studies, six pre-post studies, and four single-arm studies. A meta-analysis of five pre-post studies showed that after infection with COVID-19, sperm concentration (I2 = 0; P = 0.003), total sperm count (I2 = 46.3%; P = 0.043), progressive motility (I2 = 50.0%; P < 0.001), total sperm motility (I2 = 76.1%; P = 0.047), and normal sperm morphology (I2 = 0; P = 0.001) decreased. Simultaneously, a systematic review of 13 studies found a significant relationship between semen collection time after infection, inflammation severity, and semen parameter values, with fever having only bearing on semen concentration. Furthermore, there was no significant difference in sex-related hormone levels before and after infection in mild/asymptomatic patients. Mild/asymptomatic COVID-19 infection had a significant effect on semen quality in the short term. It is recommended to avoid initiating a pregnancy during this period of time.

ABSTRACT Introduction: Muscle fatigue bothers athletes, affecting training level and competitive performance, it also has a great impact on the physical health of athletes, predisposing them to accidents and an early termination of their career. Relieving sports fatigue is the focus of research in the field of sports health nowadays. Objective: Study the effect of acupuncture and moxibustion rehabilitation combined with physical training on sports fatigue. Methods: A controlled experiment was used. The experimental group used acupuncture and moxibustion combined with exercise training, while the control group used acupuncture and moxibustion. The same group of doctors performed the acupuncture and moxibustion treatment according to the actual situation of the patients, and they tested the changes in VAS pain score and PRI pain score. Results: The VAS pain score in the experimental group was 7.88 points before the procedure and 2.96 points after the sixth week of the procedure. The control group score was 7.67 before the start and 5.03 after training. The total PRI pain score in the experimental group was 6.52 points before training and 2.05 points in the sixth week of training. The control group scored 6.66 before the procedure and 3.89 in the sixth week. Conclusion: The combination of training and exercises can achieve a better rehabilitation effect compared to the isolated treatment of acupuncture and moxibustion. Level of evidence II; Therapeutic studies - investigation of treatment outcomes.


RESUMO Introdução: A fadiga muscular incomoda os atletas, afetando o nível de treinamento e o desempenho competitivo, também tem um grande impacto sobre a saúde física dos atletas, predispondo a acidentes e a um término precoce da carreira. Aliviar a fadiga esportiva é o foco da pesquisa no campo da saúde esportiva nos dias de hoje. Objetivo: Estudar o efeito de reabilitação pela acupuntura e moxabustão combinada com o treinamento físico sobre a fadiga esportiva. Métodos: Foi utilizado um experimento controlado. O grupo experimental usou acupuntura e moxabustão combinadas com treinamento com exercícios, enquanto o grupo de controle usou acupuntura e moxabustão. O mesmo grupo de médicos realizou o tratamento de acupuntura e moxabustão de acordo com a situação real dos pacientes, e testaram as alterações do escore de dor VAS e do escore de dor PRI. Resultados: A pontuação de dor VAS no grupo experimental foi de 7,88 pontos antes do procedimento e 2,96 pontos após a sexta semana de procedimento. A pontuação do grupo de controle foi 7,67 antes do início e 5,03 após o treinamento. A pontuação total de dor PRI no grupo experimental foi 6,52 pontos antes do treinamento e 2,05 pontos na sexta semana de treinamento. A pontuação do grupo de controle foi de 6,66 antes do procedimento e 3,89 na sexta semana. Conclusão: A combinação do treinamento com exercícios pode alcançar um melhor efeito de reabilitação em comparação com o tratamento isolado de acupuntura e moxabustão. RESUMEN Introducción: La fatiga muscular molesta a los deportistas, ya que afecta al nivel de entrenamiento y al rendimiento competitivo, además de tener un gran impacto en la salud física de los atletas, predisponiendo a accidentes y al cese prematuro de la carrera. En la actualidad, el alivio de la fatiga deportiva es objeto de investigación en el campo de la salud deportiva. Objetivo: Estudiar el efecto de la rehabilitación mediante acupuntura y moxibustión combinada con entrenamiento físico sobre la fatiga deportiva. Métodos: Se utilizó un experimento controlado. El grupo experimental utilizó acupuntura y moxibustión combinadas con entrenamiento físico, mientras que el grupo de control utilizó acupuntura y moxibustión. El mismo grupo de médicos realizó el tratamiento de acupuntura y moxibustión de acuerdo con la situación real de los pacientes, y comprobó los cambios en la puntuación del dolor según VAS y la puntuación del dolor según PRI. Resultados: La puntuación VAS del dolor en el grupo experimental fue de 7,88 puntos antes del procedimiento y de 2,96 puntos después de la sexta semana del procedimiento. La puntuación del grupo de control fue de 7,67 antes del inicio y de 5,03 después del entrenamiento. La puntuación total del dolor PRI en el grupo experimental fue de 6,52 puntos antes del entrenamiento y de 2,05 puntos en la sexta semana de entrenamiento. La puntuación del grupo de control era de 6,66 antes del procedimiento y de 3,89 a la sexta semana. Conclusión: La combinación de entrenamiento con ejercicios puede lograr un mejor efecto rehabilitador en comparación con el tratamiento aislado de acupuntura y moxibustión. ABSTRACT Introduction: Following the development of ice and snow sports tourism projects, the hidden dangers of early extensive construction have gradually emerged. It mainly manifests itself in the sports safety of ice and snow sports tourism projects. Objective: Explore the management strategy of sports safety under the snow and ice sports tourism landscape. Methods: Literature research, interviews, and questionnaires were used to analyze the development in the study area of ice and snow tourism, the tourists themselves, and regional site construction. Finally, the paper analyzes strategies to improve sports safety management in related areas from the landscape manager's point of view. Results: The development of snow and ice sports tourism projects in the study area is satisfactory; however, due to the epidemic's impact, the current situation is relatively low, showing a trend of slow improvement. In terms of sports injury risk, after optimal training and safety management, the proportion of climatic characteristics of the site was 24.81%; the proportion of the population on the ski slopes was 40.92%; the proportion of equipment cleaning and maintenance was 59.23%; the degree of congestion on the ski slopes was 41.30%; the proportion of public safety was 34.42%; the proportion of basic emergency medical treatment was 32.30%; epidemics and other infectious diseases accounted for 83.13% of the total. Follow-up is done before the start, in the process, and after the occurrence of multiple managements. Conclusion: The safety management strategy of snow and ice sports tourism combines the factors of tourists and managers, and has great practical significance, so it is worthy of promotion. RESUMO Introdução: Com o desenvolvimento de projetos de turismo esportivo no gelo e na neve, os perigos ocultos da construção extensiva precoce têm surgido gradualmente. Ele se manifesta principalmente na segurança esportiva de projetos de turismo de esportes de gelo e neve. Objetivo: Explorar a estratégia de gestão da segurança esportiva sob o panorama do turismo esportivo de neve e gelo. Métodos: Pesquisa bibliográfica, entrevistas e questionários foram usados para analisar o desenvolvimento na área de estudo do turismo de gelo e neve, e dos próprios turistas bem como a construção de locais regionais. Finalmente, do ponto de vista do gerente de paisagens, o trabalho analisa as estratégias para melhorar a gestão da segurança esportiva em áreas relacionadas. Resultados: O desenvolvimento de projetos de turismo esportivo de neve e gelo na área de estudo é satisfatório, porém, devido ao impacto da epidemia, a situação atual é relativamente reduzida, mostrando uma tendência de lenta melhoria. Em termos de risco de lesões esportivas, após treinamento otimizado e gerenciamento da segurança, a proporção de características climáticas do local foi de 24,81%; a proporção de população nas pistas de esqui foi de 40,92%; a proporção de limpeza e manutenção dos equipamentos foi de 59,23%; o grau de congestionamento das pistas de esqui foi de 41,30%; a proporção de segurança pública foi de 34,42%; a proporção de tratamento médico básico de emergência foi de 32,30%; epidemias e outras doenças infecciosas representaram 83,13% do total. O acompanhamento é feito antes do início, no processo, e após a ocorrência de múltiplas gestões. Conclusão: A estratégia de gerenciamento de segurança do turismo esportivo de neve e gelo combina os fatores de turistas e gerentes, e tem grande significado prático, por isso é digna de promoção. RESUMEN Introducción: Con el desarrollo de los proyectos turísticos de deportes de hielo y nieve, han ido apareciendo gradualmente los peligros ocultos de la construcción extensiva temprana. Se manifiesta principalmente en la seguridad deportiva de los proyectos turísticos de deportes de hielo y nieve. Objetivo: Explorar la estrategia de gestión de la seguridad deportiva en el marco del paisaje turístico de los deportes de nieve y hielo. Métodos: Se utilizó la investigación bibliográfica, entrevistas y cuestionarios para analizar el desarrollo en la zona de estudio del turismo de hielo y nieve, y a los propios turistas, así como la construcción de emplazamientos regionales. Por último, desde el punto de vista del gestor del paisaje, se analizan estrategias para mejorar la gestión de la seguridad deportiva en ámbitos afines. Resultados: El desarrollo de proyectos de turismo deportivo de nieve y hielo en la zona de estudio es satisfactorio, sin embargo, debido al impacto de la epidemia, la situación actual es relativamente baja, mostrando una tendencia de lenta mejora. En cuanto al riesgo de lesiones deportivas, tras optimizar la formación y la gestión de la seguridad, la proporción de características climáticas del lugar fue del 24,81%; la proporción de población en las pistas de esquí fue del 40,92%; la proporción de limpieza y mantenimiento del equipo fue del 59,23%; el grado de congestión de las pistas de esquí fue del 41,30%; la proporción de seguridad pública fue del 34,42%; la proporción de tratamiento médico básico de urgencia fue del 32,30%; las epidemias y otras enfermedades infecciosas representaron el 83,13% del total. El seguimiento se realiza antes del inicio, en el proceso y después de que se produzcan múltiples gestiones. Conclusión: La estrategia de gestión de la seguridad del turismo deportivo de nieve y hielo combina los factores de los turistas y los gestores, y tiene una gran importancia práctica, por lo que merece ser promovida. ABSTRACT

Central retinal artery occlusion(CRAO), also known as eye stroke, always results in acute and painless visual loss. At present, conservative treatments, such as eye massage, lowering intraocular pressure and vasodilators have little effect on reducing visual loss. Intra-arterial thrombolysis(IAT)has significantly improved prognosis in patients with acute ischemic stroke, thus IAT has been gradually applied in the treatment of CRAO. IAT injects fibrinolytic drugs directly into the ophthalmic artery by a microcatheter, and dissolves the emboli that block the central retinal artery to restore the blood flow of the retina. Theoretically, IAT may be effective for CRAO as what has been found for stroke, but existing clinical studies exhibited inconsistent results. This paper summarizes the feasibility, efficacy, and safety of IAT treatment in CRAO. It will also analyze related factors that affect the prognosis, putting forward potential development directions and providing insights for the further clinical application of IAT.

Objective To investigate the average glandular dose of digital mammography in Zigong, China, and to explore the methods of reducing average glandular dose. Methods A total of 260 patients who underwent digital mammography at six hospitals located in Zigong during the year 2022 were included in this study. Their personal information, photography position, breast compression thickness, exposure parameters, and average glandular dose were collected. Analysis of variance and correlation analysis were performed for data analysis. Results Among the 260 patients who underwent digital mammography, the average breast compression thickness was 46.3 mm and the average glandular dose was 1.70 mGy. There was a significant difference in average glandular dose between patients with different breast compression thickness(P <0.05), and average glandular dose was positively correlated with breast compression thickness. There were no significant differences in average glandular dose between patients with different photography positions and patients with different ages (P>0.05). Conclusion The average glandular dose of patients undergoing digital mammography is at an acceptable level in Zigong. The average glandular dose is impacted by the breast compression thickness, target/filter, and exposure parameters. These factors should be optimized to reduce glandular dose during digital mammography.

Objective:To investigate the disability status of patients with inflammatory bowel disease (IBD) in China and to identify the influencing factors of the inflammatory bowel disease disability index (IBD-DI).Methods:From October 1 to December 31, 2021, a total of 1 170 IBD patients were recruited from 7 IBD centers and WeChat public platforms in China. All the patients were surveyed by the IBD-DI questionnaire, which included demographic information, disease activity, medication history, treatment and surgical history. Demographic information included gender, age, income status, etc. Multiple linear regression was used to analyze the influencing factors of IBD-DI.Results:Among the 1 170 IBD patients, 746 patients (63.76%) were male and 424 patients (36.24%) were female; there were 871 cases (74.44%) of Crohn′s disease(CD), 277 cases (23.68%) of ulcerative colitis (UC) and 22 cases (1.88%) of inflammatory bowel disease undassified (IBDU). The age was 36.00 years old (29.00 years old, 45.00 years old), and the IBD-DI score was 9.00 (5.00, 15.00). The results of multiple linear regression analysis revealed that the disease activity ( β=0.65, t=22.33, P<0.001), current treatment with enteral nutrition ( β=0.09, t=3.06, P<0.001), and history of perianal surgery ( β=0.06, t=2.12, P=0.034) were influencing factors of IBD-DI in the CD patients. Disease activity ( β=0.65, t=14.37, P<0.001), household per capita annual income ( β=-0.16, t=-3.59, P<0.001), current usage of immunosuppressants ( β=0.12, t=2.66, P=0.008), current treatment with enteral nutrition ( β=0.12, t=2.57, P=0.011), and the duration of each exercise ( β=-0.12, t=-2.67, P=0.008) were influencing factors of IBD-DI in UC patients. Conclusions:Disability is common in Chinese IBD patients, and their IBD-DI were different. Disease activity is the most important factor affecting IBD-DI. The IBD-DI is higher in IBD patients receiving enteral nutrition treatment, CD patients with a history of perianal surgery and UC patients with current usage of immunosuppressants. However, household per capita annual income and the duration of each exercise are negatively correlated with IBD-DI in UC patients.

Introducción: En el contexto de la pandemia de COVID-19, la infección por Enterobacterales resistentes a carbapenémicos productores de carbapenemasas (ERC-PC) se convierte en un problema de salud pública desafiante a nivel mundial. Los médicos desempeñan un papel fundamental en el manejo clínico de las infecciones por patógenos resistentes a carbapenémicos a nivel hospitalario y por ende en el control de estas. Sin embargo, no existen estudios sobre el conocimiento y prácticas de estos sobre ERC-PC en Cuba. Objetivo: Abordar conocimientos y prácticas sobre las infecciones por ERC-PC, su manejo terapéutico, prevención y control en médicos cubanos en un hospital terciario. Métodos: Se realizó un estudio observacional (descriptivo, de corte transversal) y se aplicó un cuestionario específico elaborado a 70 médicos asistenciales. Resultados: Hubo variabilidad en las opiniones sobre las ERC-PC y un conocimiento medio sobre estas en la mayoría de los encuestados. El 58,6 % de los médicos tenía experiencia en el manejo clínico de la infección por ERC-PC. Las brechas de conocimiento encontradas estuvieron asociadas a la escasa experiencia práctica en el manejo de casos. Hubo diferencia estadísticamente significativa en el nivel de conocimientos sobre los ERC-PC en los médicos con experiencia en el manejo de los ERC-PC en comparación con los médicos sin experiencia (p = 0,039). Conclusiones: Es necesario establecer un plan de formación continua en la temática para mejorar el desempeño de los profesionales de la salud en el control de las infecciones por ERC-PC y en el uso optimizado de los antibióticos.


Introduction: In the context of COVID-19 pandemic, carbapenemase-producing carbapenem-resistant Enterobacterales (CP-CRE) infection becomes a global public health threat. Medical doctors play a fundamental role in the clinical management and control of infections caused by carbapenem-resistant pathogens. However, in Cuba there are not previous studies on the knowledge and practice of medical doctors about CR-CRE. Objective: To study the knowledge and practice of Cuban medical doctors in a tertiary hospital about CR-CRE infections, their clinical management, prevention and control. Methods: It was conducted an observational study (descriptive, cross-sectional) and an specific questionnaire to 70 medical doctors was applied. Results: There was variability on the opinions about CP-CRE and average knowledge in the majority of the respondents. 58.6% of the medical doctors had some experience in the clinical management of CP-CRE infections. Knowledge gaps were associated with the limited practical experience in the management of cases. There was statistically significant difference in the level of knowledge about CP-CRE of medical doctors with experience in the management of CP-CRE compared to inexperienced medical doctors (p = 0.039). Conclusions: It is necessary to establish a continuous training plan on this topic to improve the performance of health professionals in the control of CP-CRE infections and in the appropriate use of antibiotics.

ABSTRACT

Esta revisión tuvo como propósito explorar la distribución de serotipos y la resistencia antimicrobiana de Streptococcus pneumoniae en la población pediátrica de China a partir de literatura publicada en los últimos seis años. Se realizó una revisión de alcance a partir de PubMed y dos bases de datos de China: CNKI y WanFang Data. Del total de 196 artículos extraídos, se seleccionaron 14 estudios para esta revisión. Hay 13 artículos que analizan la distribución de serotipos de Streptococcus pneumoniae; los serotipos más frecuentemente registrados son: 19F, 19A, 23F, 14 y 6B. Hay 11 artículos que analizan la resistencia antimicrobiana de Streptococcus pneumoniae, la prevalencia de no susceptibles a la penicilina se encuentra en el rango de 0 % a 95,7 %. Los aislados son muy resistentes a eritromicina, clindamicina, tetraciclina y trimetoprima-sulfametoxazol; son resistentes a penicilina en meningitis neumocócica pero son sensibles a penicilina en otras enfermedades neumocócicas, además, son muy sensibles a levofloxacina, vancomicina y Linezolid. Se concluye que la vacuna antineumocócica conjugada 13 tiene alta cobertura en los serotipos de Streptococcus pneumoniae en los niños de China continental, por eso se recomienda su inclusión en el programa de vacunación infantil; al mismo tiempo, se debe tener en cuenta la aparición de la sustitución de serotipos. Por eso, se deben incluir más pacientes pediátricos o niños en las investigaciones, especialmente los menores de cinco años. Es necesaria una vigilancia de alta calidad a largo plazo sobre la distribución de serotipos y resistencia antimicrobiana de Streptococcus pneumoniae para el desarrollo de la prevención de enfermedades neumocócicas.


This review aimed to explore the serotype distribution and antimicrobial resistance of Streptococcus pneumoniae in the Chinese pediatric population based on literature published in the last six years. A scoping review was performed using PubMed and two Chinese databases: CNKI and WanFang Data. Of the total of 196 articles extracted, 14 studies were selected for this review. There are 13 articles that analyze the distribution of Streptococcus pneumoniae serotypes, the most frequently registered serotypes are: 19F, 19A, 23F, 14 and 6B. There are 11 articles that analyze the antimicrobial resistance of Streptococcus pneumoniae, the prevalence of non-susceptible to penicillin is in the range of 0% to 95.7%. Isolates are highly resistant to erythromycin, clindamycin, tetracycline, and trimethoprim-sulfamethoxazole; they are resistant to penicillin in pneumococcal meningitis but are sensitive to penicillin in other pneumococcal diseases, in addition, they are very sensitive to levofloxacin, vancomycin and Linezolid. It is concluded that the pneumococcal conjugate vaccine 13 has high coverage in Streptococcus pneumoniae serotypes in children from mainland China, therefore its inclusion in the childhood vaccination program is recommended; at the same time, the occurrence of serotype substitution should be taken into account. Therefore, more pediatric patients or children should be included in research, especially those under five years of age. Long-term, high-quality surveillance of the serotype distribution and antimicrobial resistance of Streptococcus pneumoniae is necessary for the development of pneumococcal disease prevention.

ABSTRACT

Objective:To investigate the clinical features of IgD multiple myeloma (MM) and the effect and prognosis of daratumumab-based combination therapy.Methods:The clinicopathological data of a IgD MM patient with disease progression and extramedullary infiltration treated with daratumumab in the Affiliated Hospital of Qingdao University in December 2019 were retrospectively analyzed.Results:The 74-year-old woman was diagnosed as IgD MM by bone marrow aspiration and immunofixation electrophoresis. The patient was given VD (bortezomib, dexamethasone), RD (lenalidomide, dexamethasone) and ID (ixazomib, dexamethasone) regimens. In June 2020, the patient developed multiple subcutaneous nodules, and she was assessed as progressive disease with extensive extramedullary infiltration. After treated with daratumumab-PAD (liposomal doxorubicin, bortezomib, dexamethasone) regimen, the patient's subcutaneous nodules were significantly reduced and partially disappeared, and the general condition was significantly improved. But the patient was in a cachexia state and finally died of the irregular treatment and disease progression.Conclusions:IgD MM has a low incidence and a short survival period, and there is no uniform standard treatment. The early application of daratumumab combined with proteasome inhibitors, immunomodulators, cytotoxic drugs and hematopoietic stem cell transplantation may improve the overall survival of patients.

ABSTRACT

Objective:To study the personality development of obsessive-compulsive disorder (OCD) from the perspective of memory-tracing.Methods:From January 2016 to May 2017, totally 103 patients with OCD (patient group) and 88 normal subjects (control group) matched with age, gender, and educational level in the national urban population norm database of Wang Weidong memory-tracing personality developmental inventory (WMPI) were included.WMPI was used to evaluate and compare all subjects.SPSS 22.0 software and nonparametric test were used for statistically analysis, and independent sample t-test and nonparametric test were used for data camparison. Results:1.In terms of external influencing factors of personality development: on the part of life events, the score of family affair in the patient group was higher than that in the control group( P<0.05) in childhood.In terms of upbringing, the scores of strictness (5.637±3.463) and punishment (6.275±4.565) in the patient group were higher than those ((4.341±3.092), (5.000±3.698)) in the control group in childhood( t=-2.703, -2.093, both P<0.05). 2.In the aspect of personality elements: (1) Among courage subscales, the scores of natural fear (7.686±3.441) and adaptability (15.000±5.321) in the patient group were higher than those((6.023±3.991), (12.841±6.070)) in the control group in childhood ( t=-3.085, -2.613, both P<0.05) .For all three stages, interpersonal fear scores in the patient group ((20.284±8.255), (22.804±7.458), (22.725±7.145))were all higher than those ((16.205±7.937), (19.841±6.319), (18.364±6.277))in the control group( t=-3.458, -2.929, -4.437, P<0.01). (2) Among interpersonal relationship, the dependence dimension scores were higher in the patient group (10.804±3.621) than those (8.830±4.850) in the control group during childhood( t=-3.205, P<0.01). (3) Among sex development, the scores of heterosexual communications (11.941±4.878), love concept (15.098±4.180) and sexual concept (8.892±2.988) were higher in the patient group than those ((9.125±5.040), (11.761±5.202), (6.943±3.288)) in the control group in adolescence(all P<0.01). (4) Among ego, the score of self-care dimension in the patient group (6.465±2.890) was higher than that in the control group(4.239±2.861) in childhood ( P<0.01). In the dimension of autonomy, the scores of the patient group ((10.772±2.694), (11.347±2.621)) were higher than those in the control group ((8.011±4.039), (9.818±2.693)) in childhood and adolescence (both P<0.01). (5) Among the way of thinking, the score of absolute thinking dimension in the patient group was higher than that in control group in childhood ( P<0.01). In the dimension of cautious, the scores of patient group were higher than those in the control group in childhood and adolescence (both P<0.01). (6)Among volition, the scores of decisive dimension in the patient group were higher than those in the control group in adolescence and youth (both P<0.01). In the dimension of consciousness, the score of patient group was higher than that in the control group in youth( P<0.01). In the dimension of insistence, the score of the patient group were higher than those in the control group in childhood and youth (both P<0.01). (7) Among worldviews, the scores of motivations, perspective of career and perspective of friendship in the patient group were higher than those in the control group in adolescence (all P<0.01). The score of value dimension in patient group was lower than that in the control group in the youth ( P<0.01). Conclusion:Patients with OCD have more strictness and punishment during their childhood in terms of upbringing.This leads to a lack of courage, poor interpersonal relationships, low self-care or autonomy, high attachment, absolutization of thinking and suppressing themself more in their childhood.In their adolescence and youth, their lack of courage, poor self-care or autonomy and the way of thinking cautious and stubborn will further aggravate and gradually show a more conservative sexual development and traditional world outlook.

Introducción: La resistencia bacteriana pone en peligro la salud y la supervivencia de los seres humanos, aumenta la carga económica de la sociedad y los pacientes. Es un fenómeno global por lo que Cuba no queda exenta. Objetivos: Exponer el impacto social y económico de la resistencia antimicrobiana desde el punto de vista filosófico y describir el rol de una medida preventiva en la contención de la resistencia antimicrobiana. Material y Métodos: Se realizó una revisión de fuentes bibliográficas que fueron localizadas mediante la base de datos Pubmed, Portal Regional de la Biblioteca Virtual de Salud y el motor de búsqueda Google Académico. Desarrollo: Se analizan los aspectos sociales, económicos y éticos relacionados con la resistencia bacteriana y se ejemplifica una medida preventiva en la contención de la resistencia antimicrobiana. Además, se analiza la relación entre fármacos antibacterianos, resistencia bacteriana y medidas de prevención y control desde el punto de vista de ciencia-tecnología-sociedad. Conclusiones: La sociedad humana se desarrolla y progresa constantemente bajo la promoción de la ciencia y la tecnología. En pocas décadas, los antibióticos han pasado de ser "drogas milagrosas de gran impacto para la salud" a ser "un recurso no renovable en vías de extinción". Se deben adoptar las acciones pertinentes para frenar el desarrollo de la resistencia bacteriana con un enfoque multisectorial. Se requiere una gobernanza, optimización del uso de antibióticos, apoyos de políticas de salud y un fortalecimiento de los programas de prevención y control de infecciones(AU)


Introduction: Bacterial resistance endangers the health and survival of human beings and increases the economic burden on society and patients. It is a global phenomenon; therefore, Cuba is not exempted from it. Objective: To present the social and economic impact of antimicrobial resistance from a philosophical point of view as well as to describe the role of a preventive measure to stop antimicrobial resistance. Material and Methods: A review of bibliographic sources was carried out in databases such as PubMed and the Regional Portal of the Virtual Health Library; Google Scholar search engine was also used. Development: Social, economic and ethical aspects related to bacterial resistance are analyzed. A preventive measure to stop antimicrobial resistance is described. In addition, the relationship between antibacterial drugs, bacterial resistance and prevention and control measures is analyzed from the point of view of science-technology-society. Conclusions: Human society is constantly developing and progressing under the promotion of science and technology. In just a few decades, antibiotics have gone from being "miracle drugs of great impact on health" to being "a non-renewable resource in danger of extinction". Necessary measures such as the optimization of the use of antibiotics, a health policy support, and a health strategy for the prevention and control of infections must be taken to stop the development of bacterial resistance(AU)

ABSTRACT

Objective: To observe the therapeutic effect of sinew-regulating bone-setting manipulations for knee osteoarthritis (KOA) model rabbits and its impacts on the chondrocyte apoptosis rate and the levels of interleukin (IL)-1β and nitric oxide (NO). Methods: According to the random number table method, 30 New Zealand white rabbits were divided into a normal group (n=9) and a modeling group (n=21). Rabbits in the modeling group were used to establish KOA models with the modified Hulth method. At the 8th week, three rabbits were sacrificed to verify the model and the remaining 18 rabbits were randomly divided into a model group (n=9) and an intervention group (n=9). Rabbits in the normal group and model group were bred routinely without any intervention. Rabbits in the intervention group were treated with the sinew-regulating bone-setting manipulations, 10 min/time, once every other day for a total of 20 times. The Lequesne MG knee function rating was used to evaluate the behavioral differences of the rabbits in each group. The Pelletier score was used to evaluate the general changes of the rabbits. The Mankin score was used to evaluate the pathology of knee cartilages. The enzyme-linked immunosorbent assay and nitrate reductase methods were used to determine the levels of IL-1β and NO in serum and synovial fluid of each group, respectively. In situ terminal deoxynucleotidyl transferase-mediated nick and labeling method was used to determine the apoptosis of chondrocytes in each group. Results: Compared with the normal group, the scores of Lequesne MG, Pelletier and Mankin, and the levels of IL-1β and NO in the model group were increased (P<0.05), which indirectly indicated the success of the model. Compared with the model group, the scores of Lequesne MG, Pelletier and Mankin, IL-1β and NO levels, and chondrocyte apoptosis rate of the intervention group were decreased, and the differences were statistically significant (P<0.05). Conclusion: The sinew-regulating bone-setting manipulations can reduce the levels of IL-1β, NO, and chondrocyte apoptosis rate, and delay the articular cartilage degeneration, therefore, having a good therapeutic effect on KOA.

ABSTRACT

A retrospective analysis of 7 patients of multiple myeloma (MM) with initial manifestation of bleeding and coagulation abnormalities were performed. Clinical manifestations, laboratory and imaging examinations were collected. The activity of coagulation factors was measured before the treatment. Single factor X deficiency was seen in one patient. Two cases had factor Ⅶ deficiency, while four other patients had multiple factor deficiency. The time from onset of symptoms to diagnosis ranged from 2 to 10 months. After anti-MM treatment started and plasma or coagulation factors were transfused, the prolonged coagulation time returned to normal from 28-84 days. Most of these patients presented large, deep and multiple sites of hematoma, which caused concerns of bone marrow puncture and may direct to other differential diagnoses. This is helpful to improve physicians′ understanding of the special clinical characteristics in MM patients.

ABSTRACT

According to the 2020 world cancer data report, the prevalence of prostate cancer ranks second in male malignant tumors and the mortality fifth. In China, due to the poor living and eating habits, the prevalence of prostate cancer is still rising, and advanced prostate cancer can adapt to and tolerate androgen castration related drugs through a variety of mechanisms, which often indicates poor therapeutic outcomes in the late stage. Houttuyniae Herba, a medicinal plant of family Saururaceae, has been officially identified both as food and medicine by the National Health Commission. The existing studies have shown that Houttuyniae Herba inhibits the proliferation, migration, and invasion of prostate cancer and induces its apoptosis and cell cycle arrest via multiple targets, links, and pathways. These anti-tumor activities can also be observed in breast cancer, leukemia, gastric cancer, colorectal cancer, and other tumor cells. Such activities are mainly related to the inhibited expression of cyclin and cyclin-dependent kinase, regulation of apoptosis-related proteins, activation of apoptosis-related pathways, reduced secretion of matrix metalloproteinase, inhibition of activator of transcription 3 (STAT3)/Zinc finger protein (Snail) / transcription factor twist signaling pathway, and antagonization of growth factor receptor, androgen, inflammation, and oxidative stress in promoting the occurrence and development of prostate cancer. In addition, Houttuyniae Herba is able to alleviate diabetes, regulate blood glucose-lowering pathways, and boost immunity by inhibiting the protein kinase B (Akt)/mammalian target of rapamycin (mTOR) and mitogen-activated protein kinase (MAPK) signaling pathways, thus suppressing the occurrence and progression of prostatic cancer. In conclusion,Houttuyniae Herba can be used as a potential drug against prostate cancer.

ABSTRACT

Objective:To investigate the expression of inositol 1, 4, 5-triphate receptor (IP 3R)-glucose-regulated protein 75 (Grp75)-voltage dependent anion channel 1 (VDAC1)-mitochondrial Calcium uniporter (MCU) Calcium axis molecules in proteinuria and to explore its upstream regulator. Methods:Sixteen Sprague Dawley rats were divided into control group (6 rats) and Adriamycin (ADR) group (10 rats). Nephropathy rat model was established by single injection of ADR through tail vein.The glomerular expression of IP 3R, Grp75, VDAC1, MCU and the activation marker of mammalian target of Rapamycin complex 1 (mTORC1) were analyzed by immunohistochemical staining.In cultured mouse podocyte, ADR was used to induce podocyte injury, and the Everolimus of different concentrations was applied for intervention.The expression of the Calcium axis molecules and apoptosis marker was analyzed. Results:Compared with control group, the glomerular expression of IP 3R (0.02±0 vs.0, P<0.001), Grp75 (0.04±0 vs.0, P<0.001), VDAC1 (0.04±0 vs.0.01±0, P<0.001), and MCU (0.05±0.01 vs.0.01±0, P<0.001) were significantly increased in ADR-induced nephropathy rats, and the activation marker of mTORC1 (0.57±0.01 vs.0.18±0, P<0.001) was increased as well.In cultured mouse podocytes, compared with control group, the expression of Grp75 (1.89±1.17 vs.0.16±0.08, P=0.001), VDAC1 (1.59±0.34 vs.0.20±0.07, P=0.006), and MCU (1.56±0.38 vs.0.46±0.35, P=0.014) were obviously increased in ADR induced podocytes, and the activation marker of mTORC1 (2.12±0.08 vs.0.39±0.09, P<0.001) was also increased.Compared with the ADR induced podocytes, the expression of Grp75 (0.26±0.20 vs.1.89±1.17, P=0.001), VDAC1 (0.40±0.26 vs.1.59±0.34, P=0.014) and MCU (0.60±0.32 vs.1.56±0.38, P=0.029) in podocytes treated with ADR and 1.0 nmol/L Everolimus were remarkably decreased, accompanied with the decrease of mitochondrial calcium [(2 664.00±140.57) U vs.(3 025.16±180.92) U, P=0.023] and apoptosis marker cleaved Caspase-3 (0.55±0.28 vs.1.48±0.45, P=0.011). Conclusions:The over-production of IP 3R-Grp75-VDAC1-MCU Calcium axis molecules accompanied with the hyper-activation of mTORC1 was involved in ADR induced nephropathy rats.mTORC1 inhibitor decreased the expression of Calcium axis molecules in mouse podocytes, which might involve in the mechanism of mTORC1 inhibitor′s effects on podocyte.
